ENTRY FORM INFORMATION Registration: 12:00 Noon Type: Scramble - Four person teams Fee: $100 per player (Includes Dinner) Bring your own four person team or register as an individual. Field will be limited to first 80 players. Trophies will be awarded to the First, Second, and Third place teams. A team roster, and sponsor sheet will be issued to each player. Dinner buffet, award presentation, and door prize drawing will be immediately after the completion of golf. No metal spikes are allowed at Sand Springs Golf Course. Anyone wishing to attend Dinner only may do so at $25 per person. Please RSVP on the entry form. |
